| 内容記述 | 本研究の目的は、医療的ケアを必要とする在宅の障害児を支援する専門職が、現在の支援の現状や多職種連携に対してどのような認識をもっているかを明らかにし、今後必要となる対策について考察することである。医療的ケアを必要とする障害児の在宅医療支援経験が3年以上ある保健師・看護師・医師・社会福祉士13人にインタビュー調査を行い、2つの分析視点に沿って質的記述的分析を行った。1つ目は、各専門職が認識している役割機能における成果と困難感であり、専門職ごとに認識されている役割機能と役割遂行上のジレンマを示す6つのカテゴリーを見出した。2つ目は、支援するうえで感じる課題と要望であり、【小児を支援するうえでの施設・制度・根拠の不足】、【医療と福祉の制度の違いから生まれる専門職間の戸惑い】、【情報共有を困難にする個人の認識や施設の背景】の3つのカテゴリーがあった。これらの結果から、今後必要となる対策として、小児の在宅医療における社会資源を拡大するための制度の充実の必要性や、ルールの異なる医療と福祉を小児の特徴を捉えてつなぐコーディネートのあり方、情報共有を強化するための連携の仕組みや教育的機能について検討していく必要性が考えられた。 | |||||||||

| 内容記述 | This study aimed to clarify how professionals support children with disabilities requiring medical care at home and recognize the current state of support and multioccupational collaboration，and to consider the necessary measures． Thirteen professionals，including public health nurses，nurses，doctors，and social workers who provided medical assistance to children with disabilities over three years were interviewed．This research was used qualitative descriptive analysis along two analysis perspectives．The first item includes achievements and sense of difficulty in the role/function that each professional recognizes and is found six categories into the following：each role/function recognized by professionals，dilemma for performing the role/function． The second item includes the challenges and requests encountered in supporting children with disabilities and includes the following three categories：lack of institution，system，and evidence for supporting children with disabilities，confusion between professionals due to differences in support approaches with medical andwelfare institutions，and personal recognition and facility background that make information sharing difficult． From these results，it is necessary to improve the system for expanding social resources in home medical care for children，and how to coordinate medical care and welfare with different rules by capturing characteristics of children，and to consider the linkage mechanism and educational functions to enhance information sharing． |
